What you will do:

- Conduct comprehensive occupational therapy assessments to identify equipment and adaptation needs for adults in the community.
- Develop and implement person-centred plans, promoting independence, safety, and quality of life.
- Provide recommendations for equipment and adaptations to facilitate daily living activities.
- Collaborate with contractors and stakeholders to ensure efficient and effective implementation of necessary adaptations.
- Assess and develop strategies for safe moving and handling practices to reduce risks for both carers and service users.
- Work closely with multidisciplinary teams, including social workers and healthcare professionals, to ensure cohesive care delivery.
- Communicate with service users, their families, and carers to ensure their involvement and understanding of the care process.
- Maintain thorough and accurate records of assessments, interventions, and outcomes.

Requirements:

- Must be registered with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
- Possess a valid UK driving licence and access to a car due to the community-based nature of the role.
- Hold a degree in Occupational Therapy or an equivalent qualification.
- Previous experience in a similar role within adult social care is desirable.
- Strong assessment and intervention skills with the ability to work both independently and collaboratively.
- Excellent communication and interpersonal skills to effectively engage with service users and professionals.
