About the Role:
We are seeking a dedicated and compassionate Occupational Therapist to join our Adult Social Care Service. In this role, you will assess and support individuals to maintain their independence and improve their quality of life. You will work closely with service users, carers, and a variety of professionals to develop personalized, outcome-focused care plans.
Key Responsibilities:
- Conduct holistic assessments to identify the needs of individuals and develop appropriate care plans.
- Provide practical support, equipment, and adaptations to help individuals maintain their independence.
- Deliver therapeutic interventions and rehabilitation programs in line with best practice guidelines.
- Collaborate with multidisciplinary teams and external partners to ensure coordinated and effective service delivery.
- Maintain accurate and up-to-date records and provide reports as required.
To be successful in this role, you will need:
- A recognized degree or diploma in Occupational Therapy.
- Valid HCPC registration.
- Experience working within a social care setting, ideally supporting adults.
- Strong communication and interpersonal skills to engage with service users, carers, and professionals.
- The ability to manage a caseload, prioritize, and organize your work effectively.
- A commitment to promoting independence, dignity, and empowerment for individuals.
