Pay Rate: £36 per hour (assignment rate)
Shift Pattern: 37.5 per week. Monday – Sunday. Mixed Shift Pattern - 08:00 am - 16:00 pm, 09:00 am - 17:00 pm, 12:30 pm - 20:30 pm. Weekends 08:00 am - 20:30 pm.

Duties:
- Be practising as a qualified ACP within the Joint Emergency Team Urgent Community Response Team, in order to provide patient-centred high-quality care
- Support a new way of working by practising as a member of the integrated multi-disciplinary Urgent Community Response (UCR) team seeing patients in their usual place of residence, providing a rapid assessment service, specialising in frailty and elderly care, at the interface between primary and secondary care
Requirements of the Senior Nurse/Advanced Care Practitoner:
- Be a qualified Healthcare Professional (i.e. Nurse, Paramedic, Physiotherapist, Pharmacist
- Hold current professional registration with respective professional body NMC/ HCPC
- Hold a MSc /PGDip in Advanced Clinical Practice or equivalent credentialed programme
- Hold a Leadership qualification
- Non-medical Prescribing V300 course
- Experience working within health care services at an advanced level of practice, and demonstrable experience as a qualified advance clinical practitioner
- Skills of assessment, examination, diagnosis, and treatment within an agreed scope of practice
- Have a full UK driving license and access to a car
