What You Will Do:
- Conduct specialist assessments, diagnoses, and develop tailored treatment plans for patients with acquired communication and/or swallowing disorders, including individuals with complex physical, cognitive, and behavioural challenges.
- Utilise expert clinical reasoning to set patient-focused goals and formulate treatment plans, working both individually and in interdisciplinary teams.
- Take on a key-worker role for specific patients, ensuring continuity of care, smooth transitions during admission and discharge, and clear communication with care managers.
- Advocate for patients with complex communication impairments, supporting them in understanding and consenting to treatment options.
- Provide guidance and support to families and carers, discussing the implications of Speech & Language Therapy (SLT) diagnoses and treatment.
Requirements:
- HCPC registration is required.
- A minimum of two years' experience in Speech & Language Therapy.
- Car driving is preferable, due to the nature of the role.
