Fantastic opportunity for an Approved Social Worker within the Older Persons and Physical Disability Service in Southwark, UK, offering a competitive hourly rate of £32. This is an ongoing locum position available for Full Time or Full-Time/Part-Time engagement. Initially a three-month role, there's potential for an extension as permanent recruitment progresses.

What you will do:

  • Manage a varied caseload and conduct comprehensive Care Act Assessments
  • Undertake unscheduled reviews of care and complete carer assessments
  • Present care packages for funding approval
  • Engage community resources to bolster service user and family independence
  • Conduct Mental Capacity Assessments and make Best Interest Decisions
  • Arrange care home placements from the community
  • Stabilise complex cases for transfer to a long-term team
  • Administer initial adult safeguarding concerns
  • Provide short-term intensive assessment periods
  • Work collaboratively with NHS, ICB, housing, and community agencies
  • Collaborate with Occupational Therapy staff
  • Employ creative problem-solving and strong communication skills

Requirements:

  • 2+ years experience post ASYE (Assessed and Supported Year in Employment), (London MOC)
