What you will do:
- Manage a varied caseload and conduct comprehensive Care Act Assessments
- Undertake unscheduled reviews of care and complete carer assessments
- Present care packages for funding approval
- Engage community resources to bolster service user and family independence
- Conduct Mental Capacity Assessments and make Best Interest Decisions
- Arrange care home placements from the community
- Stabilise complex cases for transfer to a long-term team
- Administer initial adult safeguarding concerns
- Provide short-term intensive assessment periods
- Work collaboratively with NHS, ICB, housing, and community agencies
- Collaborate with Occupational Therapy staff
- Employ creative problem-solving and strong communication skills
Requirements:
- 2+ years experience post ASYE (Assessed and Supported Year in Employment), (London MOC)
