What you will do:

- Provide a professional social work service to adults with mental health conditions, supporting their families and carers as per the Care Act 2014 and Mental Health Act 1983.
- Engage with individuals, families, groups, and communities to assess and use a range of interventions that promote independence.
- Demonstrate expert practice in complex situations, striking a balance between support and control, and liaising with various professionals at different levels.
- Develop support plans utilizing a strength-based, asset-based approach, encouraging co-production with residents and offering informed explanations of Personal Budgets and Direct Payments.
- Promote well-being and independence, enabling individuals and carers to retain skills and confidence.
- Facilitate contingency planning with families, ensuring they understand the NHS Continuing Health Care process.
- Ensure compliance with relevant legislation, conducting advocacy services and reviews, including mental capacity assessments.
- Provide supervision and guidance to less experienced social workers, fostering a culture of continuous learning and development.
