What you will do:
- Conduct holistic assessments of individuals and their carers’ needs, focusing on complex cases.
- Manage your caseload, using legal and procedural authority to protect at-risk individuals.
- Develop and support care plans that promote wellbeing, dignity, and independence.
- Undertake risk assessments and intervene in situations of conflict or neglect.
- Contribute to the safeguarding of adults at risk and ensure compliance with relevant legislation.
- Work collaboratively within a multi-disciplinary team and duty rota as needed.
- Utilise IT systems to maintain accurate records and data management.
