What you will do:
- Conduct assessments and act as a care navigator for individuals presenting with eligible social care needs, working collaboratively with health and other agencies.
- Develop and implement support plans in consultation with carers, advocates, and partner agencies to address identified needs.
- Manage and monitor caseloads efficiently, ensuring adherence to timeframes and standards.
- Undertake capacity and best interest assessments and lead safeguarding inquiries, ensuring outcomes are shared with concerned parties.
- Work in partnership with the council, multi-agency boundaries, and contribute to integrated team efforts.
- Engage in professional supervision, maintaining effective practice, reflection, and career development.
- Prepare reports and ensure they meet high-quality standards and submission deadlines.
