Location: Telford. Hybrid – client visits and office based and home.
Contract Type: Ongoing contract
Pay Rate: £34 per hour (assignment rate)

Duties:
- Manage a caseload and carry out assessments of adults and older people with various levels of complexity and some more complex assessments including housing adaption’s
- Identify risks and work to develop plans to manage or reduce risk
- Promote close and constructive relationships with adults and their families and partner agencies
- Provide Occupational therapy interventions to reduce or delay the persons needs
- Work in partnership with a range of statutory, independent and voluntary services to develop and implement care plan objectives and outcomes
- Actively participate in group meetings and support an ethos of learning and development, using evidence on which to base professional practice
- Contribute to training of students, newly qualified workers and unqualified workers
- Supervision of delegated tasks to unqualified workers
- Provide expert professional opinion and support the practice of colleagues, regarding cases and act as a resource base for other staff and students when delivering services to adults and their families
- Work to uphold quality standards and cost effectiveness of service, ensuring the most appropriate and cost effective options are considered
- Work to a range of legal options to support investigation and protection
- Making decisions in respect of care issues with adults and their families from a range of cultural backgrounds
Requirements of the Adult Social Care Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent NHS experience
- Registered with the HCPC
- Experience in adult and older peoples Occupational Therapy, including experience in assessment and care approaches to prevention and other interventions which will improve outcomes for adults and their families, including those who do not have capacity
- Have experience which demonstrates the ability to meet the requirements of the Professional Capabilities Framework
- A person centred approach reflects the skill set of the Occupational Therapist and their ability to address health and social care needs
- Car driver
