The North Locality Team is thrilled to offer an exciting opportunity for a motivated and enthusiastic individual to join our team as a Social Worker within the Locality Team in Denbighshire. This locum position, earning £38.15 hourly, is available through to 31st March 2025. Working within a dedicated team of Social Workers, Occupational Therapists, and social care staff, you'll help deliver integrated services at the Prestatyn Community Resource Team (CRT). Here, health, social care, and third sector teams work collaboratively to serve the local community. This role requires a commitment to positive and effective partnership working, aiming for realistic and desirable outcomes for individuals and their families.

What you will do:
- Conduct appropriate and proportionate assessments for individuals and families to empower them to achieve their desired outcomes.
- Collaborate actively with a multidisciplinary team, including Social Workers, Occupational Therapists, and other social care staff, to provide seamless integrated care.
- Engage in positive and effective partnership working with health, social care, and third sector teams.
- Manage and prioritise your workload efficiently, demonstrating drive, energy, and self-motivation.
- Contribute to continuing professional development activities to enhance your skills and knowledge.

This thrilling role is perfect for a Social Worker passionate about making a tangible difference in their community. The successful candidate must hold a Professional qualification in Social Work and be registered with the Care Council for Wales. This appointment is subject to Disclosure & Barring Service Check and satisfactory references.
