Salary: £36,648 - £38,223 per annum

Main Duties as a Personal Advisor:
- Pathway Plan Management: Lead and conduct comprehensive assessments and interventions related to Pathway Plans for Care Leavers. Ensure plans are regularly reviewed and updated to reflect current needs and circumstances.
- Service Knowledge and Implementation: Develop and maintain a thorough understanding of available services and resources for Care Leavers. Implement and adapt Pathway Plans to align with the evolving needs of young people.
- Support and Guidance: Provide statutory support and practical advice to Care Leavers, offering both emotional and practical assistance. Mentor and support colleagues and less experienced staff in their roles.
- Relationship Building: Build and sustain meaningful relationships with Care Leavers to understand their needs and connect them with appropriate services.
- Duty Service Provision: Contribute to and assist in managing a duty service for Care Leavers based at the Hub, ensuring effective and responsive support.
- Advocacy and Promotion: Advocate for the specific needs of Care Leavers to other departments, statutory bodies, partner agencies, and voluntary organizations. Highlight and address barriers and inequalities within the system.
- Corporate Parenting: Actively champion the needs of Care Leavers as a Corporate Parenting Champion, enhancing the Care Leaver offer and supporting their rights and opportunities.
- Comprehensive Advice and Support: Provide informed and unbiased advice on education, health, employment, training, accommodation, and other relevant services. Advocate for Care Leavers and facilitate access to necessary resources.
- Pathway Plan Coordination: Collaborate with social workers and other professionals in the preparation, review, and updating of Pathway Plans.
- Meeting Participation: Attend and lead various meetings, including Pathway Plan reviews, CLA reviews, strategy meetings, risk management meetings, transition forums, disruption meetings, court hearings, and departmental meetings. Represent the service and council professionally.
- Service Liaison: Establish strong links with relevant services such as Adult Social Care, Mental Health Services, Substance Misuse Services, and Probation Services to ensure comprehensive support for Care Leavers.
- Housing and Accommodation: Coordinate with accommodation and housing providers to secure suitable housing and tenancy arrangements for Care Leavers.
- Participation and Feedback: Lead initiatives to gather Care Leavers’ views and feedback through one-to-one sessions, group work, and evaluations. Organize and oversee the Care Leavers' Forum to facilitate their participation in service development.
- Corporate Parenting Board Engagement: Represent and support Care Leavers in engaging with the Corporate Parenting Board, advocating for their needs and interests.
- Safeguarding and Record Keeping: Adhere to local authority policies and procedures to safeguard and promote the welfare of Care Leavers. Maintain accurate case records, participate in team meetings, and engage in relevant training and development.
Key Requirements for this role:
- NVQ Level 3 in Childcare, Family Support, or a related field, or an equivalent qualification.
- Willingness to pursue further education to achieve Level 4 or higher in Childcare, Family Support, or a comparable qualification.
- Extensive experience working with care leavers, children, young people, families, and/or carers in a professional capacity.
- Proven experience in building and maintaining purposeful relationships with children, young people, families, and collaborating effectively with other professionals.
- Willingness to work outside of regular office hours as needed to meet service demands.
