What you will do:
- Facilitate safe and timely discharges from care settings collaboratively with the Continuing Health Care Team
- Conduct comprehensive assessments and reviews of individuals, ensuring all care requirements are effectively met
- Handle complex and safeguarding cases providing expert guidance and interventions
- Complete Mental Capacity Assessments and organise Best Interest meetings to ensure clients’ needs are prioritized
- Collaborate closely with the team to carry out Care Act assessments post-discharge under the Discharge to Assess D2A pathway
- Engage in safeguarding enquiries taking the lead in managing cases
- Support applications to the Court of Protection when necessary ensuring clients’ legal and welfare needs are upheld
