What you will do:
- Manage and prioritise workloads to meet deadlines while maintaining high standards of written work and effective interventions.
- Undertake assessments and reviews in a person-centred and holistic manner, focusing on individual strengths and preventative options.
- Provide support to carers and family, ensuring involvement and suitable advocacy where needed.
- Conduct Mental Capacity Assessments and make Best Interest decisions as required.
- Collaborate with providers to ensure service user outcomes are achieved and hold providers accountable.
- Maintain clear, accurate records, and ensure any changes to support arrangements are effectively actioned.
- Work within the Care Act 2014's framework, focusing on accessible, person-centred assessments.
- Fully engage in supervision and adhere to the Continuing Health Care framework.
- Collaborate with commissioning to ensure provider quality standards and address any arising concerns.
- Prioritise the safety and wellbeing of service users by managing risks and supporting positive risk-taking.
- Engage with the Haringey Safeguarding Process and statutory guidance to address concerns appropriately.
- Contribute to team meetings for improving outcomes, best practice, and service development.
- Work closely with professionals, organisations, and carers to achieve set targets and outcomes.
- Stay informed about local resources and services within the voluntary and community sectors.
- Adhere to health and safety responsibilities and maintain professional development.
