What you will do:
- Manage complex caseloads within the Older Persons and Physical Disability Service.
- Conduct comprehensive assessments including Care Act Assessments, Carers Assessments, Mental Capacity Assessments, and CHC Assessments.
- Present care packages for panel approval and coordinate care home placements.
- Stabilise complex cases and address initial safeguarding concerns effectively.
- Collaborate with partner agencies such as GPs, Housing, and Occupational Therapy staff to ensure seamless care.
- Utilise organisational IT platforms like Mosaic and Liquid Logic with proficiency.
This position offers the unique opportunity to flex your investigative prowess while engaging with diverse and stimulating cases. Based at Southwark Council's Queens Road office, the role involves minimal presence on-site with just two compulsory in-office days, affording you greater flexibility. Transport is facilitated by a team Oyster card, and you will be supported with a laptop and smartphone for efficient working.
