We are seeking an experienced professional to join our team as a Senior Practitioner - Older Adult Community Mental Health, based in the scenic location of Saint Peter Port, Guernsey. What you will do:
- Manage a caseload of patients experiencing complex mental health difficulties, offering assessments, treatment planning, and risk management.
- Deliver care to older adults with both functional and organic mental health concerns, either within the community or residential care settings.
- Coordinate seamless transitions between various departments and services, ensuring integrated care pathways.
- Conduct comprehensive memory assessments and provide post-diagnostic support, including effective signposting.
- Collaborate with other agencies to develop and implement care packages that cater to individual needs.
- Support the team manager in service delivery and step in to deputise when necessary.
- Contribute to training, supervision sessions, and development activities within the team and the larger service.
- Utilise your skills in communication effectively with both internal teams and external agencies to advocate for patients and support junior staff.
