The Role:
As a Senior Community Practitioner, you will support the delivery of specialist care to individuals with substance misuse issues, working collaboratively with clients, their families, and other professionals. You’ll provide leadership within the CDAT team, ensuring effective care planning, intervention, and service development.
Key Responsibilities:
- Conduct detailed assessments and develop tailored care plans for individuals experiencing substance misuse.
- Deliver evidence-based interventions, including harm reduction and relapse prevention strategies.
- Provide consultation and advice to colleagues, families, and partner agencies to ensure continuity of care.
- Facilitate both group therapy sessions and individual counselling using motivational and cognitive-behavioural approaches.
- Actively lead and supervise a team, ensuring professional development and adherence to high clinical standards.
- Participate in service development by contributing to audits, implementing best practices, and updating clinical policies.
- Work as part of the broader Adult Community Mental Health Services to integrate care pathways and ensure comprehensive service delivery.
You will bring:
- Registered Nurse, Social Worker, or equivalent professional qualification (e.g., NMC, HCPC, or equivalent registration).
- Postgraduate training or substantial experience in substance misuse.
- Significant experience in working with complex substance misuse cases and supporting clients with co-occurring mental health challenges.
- Proven ability to lead and support a multidisciplinary team.
- Knowledge of evidence-based substance misuse interventions and harm reduction principles.
- Strong communication, counselling, and motivational interviewing abilities.
- Organised and confident in managing caseloads and team responsibilities.
- A team-focused approach, collaborating effectively within multidisciplinary frameworks.
- Desirable Skills and Experience:
- Experience managing dual-diagnosis cases.
- Training in trauma-informed care or group therapy facilitation.
- Participation in service innovation or improvement projects.
