Main Duties:
- Manage a personal caseload of patients diagnosed with Learning Disabilities or Autism Spectrum Disorder (ASD). Conduct initial assessments for new referrals, develop appropriate care plans, and implement follow-up procedures.
- Participate in processes designed to support patients at risk of self-harm or suicide, following establishment policy and NICE Guidance.
- Attend monthly supervision sessions with your line manager and complete annual Employee Development Reviews (EDR) to evaluate targets, progress, and learning objectives.
- Utilize excellent communication and interpersonal skills, along with patience and resilience, to support patients effectively.
- Provide training and advice to staff on the care of patients with Learning Disabilities.
- Assist in organizing and conducting awareness days and workshops led by the Primary Care Mental Health Team (PCMHT), and collaborate with the team to enhance these initiatives.
- Attend and contribute to weekly Integrated Mental Health Team Referral meetings to ensure a coordinated approach in managing at-risk patients.
Requirements of the Prison Registered General Nurse:
- Valid NMC registration
- 5-year residency history in the UK
- Evidence of ongoing professional development in clinical practice
- Experience working in a secure environment
- Degree-level qualification or higher
- Experience in leading or participating in innovation and change initiatives
- Nurse Prescriber qualification
- Proven ability to contribute to a ‘value for money’ service
- Possession of a valid driving licence
