If you are HCPC registered with at least 2 years of work experience in the UK, this could be your next exciting career move. NHS experience is preferred and possessing knowledge of working with oncology and haematology patients will set you apart. Being able to work autonomously and having a background as an inpatient physiotherapist will also be advantageous. A car driver is preferable to assist with the demands of this dynamic role.

What you will do:
- Provide specialist physiotherapy assessment and interventions within the discharge planning team.
- Collaborate with the multidisciplinary team to ensure effective discharge planning and continuity of care.
- Develop and implement comprehensive discharge plans tailored to individual patient needs.
- Liaise with patients’ families and carers to ensure smooth transitions from hospital to home.
- Utilise your knowledge of working with oncology, haematology, and general inpatient cases.
