What you will do:
- Lead and develop clinical pharmacy services tailored to frailty, elderly, and stroke patients, enhancing patient care pathways.
- Collaborate with multidisciplinary teams, providing expert clinical pharmacy services in emergency care and frailty settings.
- Mentor junior pharmacists and support the strategic advancement of medicines management.
- Deliver specialized pharmacy services to the Women and Children's division, optimizing medication usage through guideline development.
- Lead the respiratory medicine pharmacy service, engaging in team meetings and contributing to patient management plans.
- Offer expert clinical pharmacy services in the cardiology department, focusing on safe and effective cardiovascular medication use.
Requirements:
- HCPC registration required.
- A minimum of two years of experience in clinical pharmacy.
- Car driving is preferable.
