Salary: £35 Hourly

This is an exciting opportunity for an experienced Occupational Therapist to join our Discharge to Assess team in the vibrant London Borough of Tower Hamlets. What you will do:

  •  Conduct Care Act assessments for individuals discharged from hospital to a step-down bed, primarily in nursing home settings.
  •  Collaborate closely with the multidisciplinary team (MDT) to devise the best support strategies for clients returning to their community homes.
  • Arrange necessary care and support to facilitate seamless transitions from the step-down facility to home.
  • Maintain accurate records and documentation for each individual's care plan and progress.
  • Engage with clients and their families to understand their needs and preferences in the discharge planning process.


Requirements:

  • HCPC registration is essential.
  •  Minimum of 2 years of experience in occupational therapy, preferably with a focus on discharge planning.
  • A car driver is preferable to facilitate travel between multiple sites and community visits.
