Job Title: Locum Occupational Therapist - Discharge to Assess
Location: London Borough of Tower Hamlets
Salary: £35 Hourly

What you will do:
- Conduct Care Act assessments for individuals discharged from hospital to a step-down bed, primarily in nursing home settings.
- Collaborate closely with the multidisciplinary team (MDT) to devise the best support strategies for clients returning to their community homes.
- Arrange necessary care and support to facilitate seamless transitions from the step-down facility to home.
- Maintain accurate records and documentation for each individual's care plan and progress.
- Engage with clients and their families to understand their needs and preferences in the discharge planning process.
Requirements:
- HCPC registration is essential.
- Minimum of 2 years of experience in occupational therapy, preferably with a focus on discharge planning.
- A car driver is preferable to facilitate travel between multiple sites and community visits.
