What you will do:

  • Focusing on the newly implemented discharge to assess process, you'll ensure smooth patient transitions from the hospital to home.
  • Conduct bedside assessments at the hospital to evaluate patients' needs and readiness for discharge.
  • Travel within North Oxfordshire, utilising your own transport, to complete home assessments aimed at promoting independence.
  • Support individuals on their reablement journey, helping them regain confidence and autonomy.
  • Carry out care act assessments, working towards tailored care plans that enhance patient outcomes.
  • Attend occasional office meetings, remaining integrated with our dynamic team, while enjoying the flexibility of remote paperwork completion.
  • To thrive in this role, you'll need to be at least two years post-qualified, HCPC registered, and a confident car driver.
