Location: Croydon
Contract Type/Length: Locum ongoing contract. Full-time, part-time will be considered
Pay Rate: £25 to £30 per hour (Assignment rate)
Main duties:
- Conduct assessments for minor equipment and adaptations (e.g., grab rails) and complete duty functions as required
- Manage a caseload of individuals with diverse and complex needs
- Work with other professionals to ensure comprehensive provision
- Accurately input information for monitoring and planning purposes
- Aim to provide an efficient and cost-effective service, utilizing appropriate resources from Social Services, Health, the Council, and community organisations
