Occupational Therapist - Band 6 within Community in Bristol UK, earning £26 hourly. This is an ongoing role with full-time opportunities, and part-time options will be considered. What you will do:
- Conduct assessments of patients in their own homes who have been referred for planned occupational therapy intervention, ensuring the highest level of care.
- Carry out initial remote assessments and triage visits, prioritizing and planning interventions appropriately.
- Collaborate with a multidisciplinary team to ensure comprehensive care for each individual.
- Provide expert advice and support to patients and their families, fostering independence and improving quality of life.
- Keep detailed records and contribute to the development of care plans tailored to each patient's needs.
Requirements:
Must be registered with the Health and Care Professions Council HCPC in the UK.
Possess at least 2 years of experience in a similar role.
A car driver is preferable to facilitate travel within the community setting.
