What you will do:
- Assess clients' needs within their living environments to develop personalised reablement plans.
- Collaborate with a multidisciplinary team to deliver tailored therapeutic interventions.
- Monitor and evaluate clients' progress, adjusting plans as necessary to achieve optimal outcomes.
- Provide guidance and support to clients' families and carers, fostering a positive environment for recovery.
- Keep detailed records and reports in alignment with professional standards and organisational policies.
Requirements:
- Applicants must be HCPC registered and hold at least 2 years of experience in Reablement services.
- A driving licence is preferable but not essential as travel to multiple sites in Southwark may be required.
