Banding/Grade: Band 5/6
Pay rate (Assignment rate): £20 - £25 per hour
Contract Length: Locum, 3-month contract
Shift Pattern: The service is operational 08:00 am - 18:30 pm, Monday - Friday and 08:00 am - 16:00 pm Saturday and Sunday. Shifts covering all of these hours are required for this role.

Duties:
- Assist the Discharge Coordinator with setting standards of care
- Responsible for managing staff within the Transfer of Care Hub (TOCH)
- Responsible for ensuring excellent clinical standards at all times / leading by example
- Provide and co-ordinate clinical care and support staff in decision making
- Assist the managers in dealing with staffing issues across the team, providing senior nurse support and guidance
- Act as a resource to facilitate a whole system approach to the transfer/discharge process and promote a positive experience for the patient
- Lead in clinical decision making especially with regard to admission avoidance within the Emergency department
- Holistically assess patients in a hospital environment
- Create a personalised plan of care for all patients once assessed
Requirements for the Discharge Nurse Practitioner role:
- Registered nurse or Therapist with evidenced relevant experience such as previous A&E, community or Nurse in charge experience
- Registered mentor
- Active NMC Registration
- Registered nurse 1st level/Therapist or educated to degree level
- Evidence of continuous development
- Know principles of infection control
- Sound clinical knowledge and skills in the support of immediate management of patients requiring clinical intervention
- Evidence experience in identifying and managing unwell patients
- Experience of multi-disciplinary working
- Able to work flexibly and cooperatively as well as autonomously
- Confident with decision making
- Confident in the MCA 2005
- Possess a working knowledge of IT software programmes and how they function, with a good standard of inputting skills
- Evidence of an awareness and understanding of national targets both in health and social care
- A willingness to further develop skills including clinical and leadership possibly through degree modules
- Sound knowledge of MCH services available in the community
- Proven and relevant experience registered nurse/Therapist
- Previous experience in A&E or community nursing
