Sanctuary Personnel are seeking a dedicated Community Nurse to join our client's Integrated Community Nursing Team, providing essential nursing care for patients in the community. As a key member of our team, you will play a vital role in promoting a positive image of community nursing and delivering high-quality care to our patients.
As a Community Nurse, you will have the following key responsibilities:
- Promoting a positive image of community nursing through professional conduct.
- Providing nursing care for community patients.
- Educating patients, carers, and new team members.
Communication and Relationships:
- Maintain confidentiality and adhere to data protection regulations.
- Maintain accurate records and communicate effectively with patients' relatives/carers.
- Conduct team meetings in the absence of the Team Leader.
- Liaise with healthcare professionals and participate in multidisciplinary team meetings.
Analysis and Judgement:
- Assess nursing aid and equipment needs and ensure provision.
- Evaluate care effectiveness and participate in collaborative practice development.
- Empower individuals and carers in treatment decision-making.
Planning and Organising:
- Plan, prioritise, and delegate care needs effectively.
Patient and Client Care:
- Conduct assessments, plan care, and support end-of-life care.
- Participate in on-call arrangements as required.
Service and Policy Development:
- Adhere to policies and contribute to educational development.
- Ensure cost-effective service delivery.
Managing Financial Resources:
- Promote efficiency in service provision.
People Management and Development:
- Develop competencies and adhere to professional codes of conduct.
- Participate in personal development and clinical supervision.
Information System Use and Management:
- Utilise IT systems effectively for data recording and communication.
- Support the implementation of relevant systems.
Involvement in Surveys and Research:
- Participate in data collection and research activities.
- Ensure adherence to performance targets and educational audits.
Freedom to Act and Autonomy:
- Adhere to policies and procedures.
Key Skills Required:
- Registered nurse with current NMC registration.
- Recent experience in community nursing.
- Willingness to undertake further study.
- Ability to prioritise, delegate, and work effectively.
- Good communication and interpersonal skills.
- Car driver with clean licence.
