Duties of the Occupational Therapist/Physiotherapist:
Intermediate Care
- Provide therapeutic interventions to support recovery and rehabilitation in a community setting.
- Collaborate with multidisciplinary teams to develop and implement care plans.
- Monitor and evaluate patient progress, adjusting treatment approaches as needed.
Community reablement and Assessment
- Conduct comprehensive assessments of individuals’ needs and capabilities to facilitate their independence.
- Design and deliver tailored reablement programmes that focus on enhancing daily living skills.
- Work closely with patients and their families to set achievable goals and support them in reaching these objectives.
Requirements of the Occupational Therapist/Physiotherapist:
- Degree or equivalent in Occupational Therapy/Physiotherapy
- Recent UK experience working as an Occupational Therapist/Physiotherapist
- Recent NHS experience working as an Occupational Therapist/Physiotherapist
- Registered with the HCPC as an Occupational Therapist or Physiotherapist
- Car Driver - Preferable
