Main Duties: 

  • Enhance Health and Wellbeing: Promote the health and wellbeing of individuals with complex diabetes, helping them achieve optimal glycaemic control through informed self-management.
  • Empower Self-Management: Support and empower individuals to manage their diabetes effectively, aiming for a normal lifestyle while reducing morbidity and premature mortality by identifying and preventing complications.
  • Educational Support: Offer education and guidance to individuals with diabetes and their carers, enhancing their understanding and management of the condition.
  • Foster Collaborative Working: Encourage and facilitate regular, effective collaboration among all healthcare professionals and stakeholders involved in diabetes care.
  • Facilitate Communication: Serve as a liaison between GP and consultant-led specialist teams to ensure cohesive and coordinated care.

Requirements of the Diabetes Specialist Nurse:

  • Must have a degree/equivalent within Nursing (RGN qualified) and an active NMC Registration (Adult)
  • Specialist Diabetes qualification (Diploma)
  • Experience of working with patients with complex diabetes.
  • Recent UK nursing experience
