What you will do:
- We are seeking a candidate with some community mental health nursing experience. Ideally community forensic experience. Transferrable skills are possible from those who have experience in other community services such as psychiatric liaison, assessment and liaison teams, early intervention, etc.

- Cover duty role as rostered.
- Cover depot clinic duty role as rostered.
- Will be allocated a caseload to case manage in the community.
- Must be able to see patients face to face in outpatient settings or in their homes.
- Commute to teamwork base daily. Travel to patients homes as required.
- Case management essential requirements are that routine face to face reviews of all patients occurs at least once every 28 days (and more frequently if needed)
- Ensure key performance indicators are met in the domains of risk assessment, risk management, care plans, physical health screens, arranging CPAs, drug and alcohol screens, carer’s engagement plans, timely submissions of social circumstances reports for tribunals and managers hearings, timely submissions of quarterly reports to the MOJ for conditionally discharged patients, submit SLP referrals, submit safeguarding concerns, submit care act assessment referral requests.
- Candidates will also be required to administer depot medication and participate in on-the-job medication competency checks.
- Candidates will be solutions orientated regarding community patients who are prescribed oral medications such as clozapine.
- Attend all daily risk zoning meetings.
- Attend clinical group supervision (e.g. reflective practice) and 1:1 supervision.
- Attend and participate in weekly MDT case management meeting.
- Rudimentary understanding of funding processes for packages of care and community funded care and support accommodation.
- Liaise with various stake holders including carers / relatives, supported housing providers, Jigsaw Team, National Probation Service, Police, MAPPA, Housing Services, Inpatient services, Social Services and other outpatient services.
- Ideally candidate to be familiar with EPJS (SLAM’s patient journey system.
Qualification:
- Registered Mental Health Nurse (RMN) with current NMC registration.
- Experience working in forensic mental health, criminal justice, or similar settings is desirable.
- Experience in risk assessment, care planning, and delivering mental health interventions.
- Knowledge of relevant mental health legislation (e.g., Mental Health Act, Safeguarding Adults/Children).
