What you will do:

  • Act as a role model to motivate team members towards innovative and high standards of practice.
  • Facilitate the development of productive working relationships throughout the Trust and with external health professionals.
  • Triage referrals and coordinate patient care through holistic assessment, planning, implementation, and evaluation.
  • Provide public health programmes aimed at improving health and reducing inequalities in the community.
  • Mentor and support pre-registration nursing students, planning their learning programmes.
  • Coordinate patients’ rehabilitation programmes and liaise with professionals to ensure timely and safe discharges.
  • Maintain up-to-date patient documentation, ensuring compliance with NMC guidelines.
