What you will do:
- Conduct thorough assessments of new referrals using advanced assessment tools such as CAARMS.
- Coordinate and manage a caseload under the Care Programme Approach, ensuring each service user has a tailor-made care plan.
- Develop and maintain therapeutic relationships with clients, adapting care plans to meet changing needs.
- Design personalised risk management plans ensuring safety and adherence to the Mental Health Act 1983.
- Administer long-acting anti-psychotic depot injections and monitor medication adherence.
- Identify and address safeguarding issues, leading investigations when necessary.
- Facilitate necessary referrals to crisis teams or hospital admissions and participate in inpatient care planning.
- Collaboratively engage with multidisciplinary teams, service users, carers, and key partner organisations to enhance service quality.
