What You Will Do:

  • Assess individual needs and develop, plan, and deliver person-centred care from initial assessment to discharge.
  • Create and review care plans, collaborating with service users, carers, and multidisciplinary teams to ensure personalized support.
  • Provide community-based support, empowering mental health service users on their recovery journey and facilitating their integration into the community.
  • Advocate for service users, ensuring their needs and rights are respected and reflected in all aspects of care.
  • Offer supervision and guidance to junior or non-qualified staff, promoting professional development within the team.
  • Collaborate with external partners like social services and healthcare providers to ensure holistic care and effective interventions.
  • Provide therapeutic interventions and direct individuals to appropriate resources and services to support their well-being.
  • Apply your knowledge of the Mental Health Act to ensure all clinical practices are aligned with legal and ethical guidelines.
