Sanctuary Personnel is currently looking for an experienced Band 6 Community Psychiatric Nurse (CPN) to take on a new ongoing contract role within the Assessment and Treatment Team.
Location: Aylesbury
Pay Rate: £26 per hour (assignment rate):
Contract Type: Locum, 6-month contract/ongoing block booking.
Working hours: Monday – Friday, 37.5 hours per week (hours flexible between 8am and 8pm). Part time will be considered.
Main Duties:
- Autonomous Care: Act as an independent practitioner, conducting specialist nursing assessments in collaboration with service users and their families/carers, and sharing findings with the wider multi-disciplinary team (MDT).
- Collaborative Care Planning: Work closely with service users and their families/carers to develop personalised treatment plans that ensure safety, promote well-being, and foster independence. This may involve partnering with external agencies.
- Deliver Evidence-Based Interventions: Provide interventions guided by NICE recommendations to support recovery and prevent relapse across various settings.
- Ongoing Care Monitoring: Regularly review treatment outcomes, assist service users in completing clinical outcome measures, and adjust care plans as needed.
- Physical Health Management: Apply knowledge of common physical health conditions to perform cardiometabolic assessments.
- Medication Expertise: Maintain extensive knowledge of medications, their side effects, and management, ensuring adherence to NMC guidelines and Trust policies.
- Consultation and Collaboration: Provide expert consultation in multi-agency meetings, forums, and services to ensure holistic care.
- Duty Provision: Contribute to the team’s duty roster, ensuring consistent service delivery.
Requirements of the Community Psychiatric Nurse:
- Degree or equivalent in Mental Health Nursing
- Experience working as a Mental Health Nurse
- NMC Registration
