Main duties:

  • Receive and scrutinise appropriateness of referrals
  • Utilise excellent interpersonal skills, ability to listen others’ views, respecting and valuing individuals frodiverse range of backgrounds.
  • Carry out assessments (including for those in crisis) in the community, in the Emergency Departmenpolice custody independently, liaising with family and professionals and taking an active role in risk-asseand safety planning. Where appropriate, promoting and enabling the use of alternatives to hospital admLiaising and working positively with members of the team, other teams within the department, and partnorganisations.
  • Work collaboratively with colleagues to empower service users and carers, maximise user independence, and ensure the protection of islanders’ human rights.
  • Recognise and address concerns about the physical health needs of service users, and how they iwith the service user’s mental health needs.
  • Encourage and empower service users to have an optimum level of responsibility for their individuaprogram of care and with their consent, where appropriate, seeking the cooperation of relatives, carers, community provisions following recovery principles and approaches.
  • Support service users to adopt recovery strategies that promote their wellbeing, healthy lifestyle, independence, and self-care.
  • Responsible and professionally accountable for a caseload as part of the crisis and assessment team.

 

Requirements of the Community Mental Health Practitioner:

  • A recognised qualification in social work, mental health nursing, or occupational therapy
  • NMC Registration
  • Experience of working in community mental health
  • Experience of working in acute mental health care
  • Knowledge of the application of the Mental Health (Jersey) Law (2016) and its Code of Practice (2018). Specialist knowledge of the different models of mental disorder
  • Specialist knowledge of the pharmacological and psychosocial interventions available for the treatment mental disorders.
  • Excellent knowledge and understanding of Health and Community Services policies and procedures
  • Excellent knowledge and understanding of the principles of safeguarding and the relevant policies and procedures for both adults and children
  • Knowledge of a range of risk assessment tools in the assessment and treatment of mental disorders.
  • Knowledge of services for people with mental health problems in Jersey
