About the Role
As a Community Mental Health Practitioner, you will play a critical role in supporting the mental health needs of adults in Jersey. Working for a highly regarded healthcare organization, your work will span community settings, emergency departments, and collaborative partnerships with local services. You’ll assess, plan, and deliver high-quality care that promotes recovery, independence, and safety.
Key Responsibilities
- Referral Management: Review and assess the appropriateness of referrals to the service.
- Comprehensive Assessments: Conduct in-depth assessments, including crisis evaluations, across diverse settings such as homes, emergency departments, and police custody.
- Risk Management: Perform risk assessments and create effective safety plans, prioritizing alternatives to hospital admission where possible.
- Collaborative Care: Work closely with families, colleagues, and partner organizations to deliver holistic, person-centered care.
- Recovery-Focused Support: Empower individuals to adopt strategies that enhance their well-being, independence, and self-care.
- Safeguarding Leadership: Ensure the safety and protection of service users by applying robust safeguarding principles.
- Caseload Management: Lead the management of a caseload as part of the crisis and assessment team, providing timely and effective interventions.
What You’ll Need
- Qualifications: A recognized qualification in social work, mental health nursing, or occupational therapy.
- Registration: Active registration with the NMC or equivalent professional body.
- Experience: Proven experience in community mental health and acute care.
- Knowledge: Deep understanding of the Mental Health (Jersey) Law 2016, safeguarding practices, and psychosocial interventions.
- Skills: Exceptional communication, risk assessment, and collaborative working abilities.
