Job Summary:

Sanctuary Personnel is seeking a Specialist Practitioner to join our client's Community Drug & Alcohol Team, delivering specialist assessments and interventions in various settings. The role includes managing medication induction and detoxification processes both in inpatient and community environments.

You will establish and maintain multi-agency and multidisciplinary links with mental health and general health services, focusing on services users' blood-borne virus treatment, investigation, and sexual health.

Relationships:

  • Responsible To: Service Manager, Adult Community Services
  • Accountable To: Associate Director of Mental Health and Adult Disability Services
  • Reports To: Community Drug and Alcohol Team Manager (CDAT)

Main Duties and Responsibilities:

Clinical:

  • Conduct assessments for patients/clients with complex care needs, including those with undiagnosed conditions, following CDAT pathways.
  • Develop, implement, and evaluate specialised care programmes.
  • Guide and advise staff in the assessment, planning, implementation, and evaluation of specialised care.
  • Perform comprehensive risk assessments and implement risk management plans.
  • Provide specialist advice in managing emergencies and acute situations.
  • Use data from various sources to guide decision-making.
  • Request clinical tests within the scope of practice to establish diagnoses.
  • Work autonomously to provide specialist assessments, recognizing and responding to issues like abuse, neglect, self-harm, and violence.
  • Act as a named care coordinator, managing care packages and coordinating multidisciplinary interventions.

Leadership/Managerial:

  • Ensure effective communication systems within and outside the team.
  • Make staff aware of organisational objectives, policies, and procedures.
  • Support staff with performance issues and developmental needs.
  • Act as a professional role model, promoting high standards and strong values.
  • Coordinate multi-agency activities and contribute to the development of CDAT’s policies and services through research, evaluation, and audit.
  • Contribute to clinical governance strategies and quality initiatives.
  • Participate in multi-agency meetings and safeguarding procedures.

Communication:

  • Communicate effectively with patients/clients and significant others, considering cultural backgrounds and cognitive functioning.
  • Manage conflict and act as an advocate for patients/clients and junior staff.
  • Maintain high standards of clinical record-keeping and provide comprehensive reports.

Training and Education:

  • Promote health and well-being and incorporate health promotion in care planning and delivery.
  • Supervise and mentor staff, students, and trainees.
  • Create a conducive learning environment and develop relevant learning programmes.
  • Maintain professional development and competence in specialist skills.
  • Contribute to the educational needs of trainees and new staff.

Continuing Professional Development:

  • Attend mandatory training and professional development activities.
  • Participate in personal review meetings and maintain professional registration.
  • Contribute to staff meetings, supervision sessions, and training courses.

Audit/Information Technology/Research Activity:

  • Conduct clinical audits to maintain practice standards.
  • Promote evidence-based practice and incorporate new initiatives.
  • Use organisational electronic information systems relevant to the role.
  • Apply research to inform evidence-based practice in substance misuse work.

General Responsibilities:

  • Continually develop within the role, with duties reviewed periodically.
  • Adhere to health and safety policies and clinical governance standards.
  • Maintain confidentiality and adhere to safeguarding policies.
  • Work with other agencies to address child protection and adult at risk issues.

Key Criteria:

Essential:

  • Professional registration (RGN/RMN) or social work qualification.
  • Evidence of CPD at Diploma level.
  • Diploma in a related field.
  • Qualified to mentor or supervise students and staff within a relevant field of mental health or social welfare practice.
  • Multi-disciplinary experience with clients, families, or groups.
  • Experience in teaching, training, and professional supervision.
  • Autonomous responsibility for clients with substance misuse issues.
  • Advanced knowledge and skills in professional practice.
  • High-level knowledge of substance misuse and addiction.
  • Effective communication skills, both oral and written.
  • IT skills and ability to work with diverse backgrounds.
  • Knowledge of relevant legislation and risk assessment.

Desirable:

  • Experience in Dual Diagnosis.
  • Presentation skills for various settings.
  • Knowledge of research design and methodology.
