Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Advanced Social Worker within the Adult Home First & Urgent Response Team to work full time based in Swindon.
The salary for this permanent Advanced Social Worker job is up to £46,549 per annum.
Main duties:
- Providing support and leadership to our expanding team of both Social Workers and Assistant Care Managers within the intermediate care services.
- A dynamic approach, with a passion for developing the service, embedding strength based practice and with support of the management team upskilling the workforce to both manage risk and provide better outcomes for the people we support.
- Undertaking Adult Safeguarding Enquiry Manager role and offering support to the team when undertaking the Enquiry Officers roles to safeguard adults with care and support needs, who are at risk of abuse or neglect, with the aim of identifying and mitigating risks to ensure the person retains choice and control over their life.
- Assess individuals who have maximised their independence through an intermediate care provision, and arrange and coordinate ongoing support in their own home, or a place they call home.
- Undertaking a strength-based approach when completing Care Act Assessments and Support Plans for individuals that are receiving reablement, pre-assessment or D2A support.
- Arrange and coordinate ongoing support (post assessment) for the individual, including Carers Assessments of relevant.
- Support to complete complex Medical capacity assessments, best interest decisions and chair best interests meetings.
- Ensuring a comprehensive handover to the net allocated team and all ongoing referrals.
- To work closely with existing primary and secondary health, together with tertiary voluntary agencies to provide holistic and individualised care plan.
Requirements of this role:
- Social work qualification with registration with Social Work England.
- Experience in managing social work caseloads and supervising others.
- Experience of community and/or group work.
- Knowledge of children’s health and social care systems, agencies, and relevant legislation.
