What you will do:
- Complete Care Act Assessments create Care Plans and arrange social care packages tailored to individual needs.
- Conduct both Placement and Community reviews for existing service users ensuring their evolving needs are met.
- Lead Safeguarding screenings enquiries and reporting facilitating positive outcomes for all involved.
- Perform Mental Capacity Assessments and make Best Interest Decisions advancing to the Court of Protection if required.
- Participate in Social Work Duty at the Civic Centre providing essential support for urgent cases and contributing once every week.
