What you will do:
- Provide an immediate response to patients at risk of hospital admission, assessing, planning, and implementing high-quality nursing care.
- Utilise the Virtual Ward model to monitor patients digitally while providing face-to-face care as required.
- Lead and manage a designated caseload of patients, ensuring that care plans are tailored and delivered to an exceptional standard.
- Work collaboratively within a multidisciplinary team to coordinate care and support patients in remaining safely at home.
- Contribute to the clinical governance agenda, attending team meetings and participating in service development projects.

Requirements for the Registered General Nurse role:
- Degree or equivalent in Nursing (RGN qualified) with active NMC Registration (Adult)
- Recent nursing experience in the UK
