As part of the community nursing team, you'll be helping housebound patients, offering your expertise as a full-time or part-time member of staff, depending on your preference.

What you will do:
- Deliver nursing care to a designated patient caseload, utilising your expertise autonomously.
- Undertake holistic assessments of both routine and complex patients to collaboratively plan care with patients and families.
- Independently provide planned care, document interventions, and escalate concerns as needed.
- Participate in daily handovers, order necessary equipment, and liaise with GPs and colleagues.
To excel in this role, you will need essential skills including driving with access to a car, proficiency in the Rio system, holistic patient assessments, insulin administration, glucose monitoring, wound care, and injections.
