Salary: £40-45 per hour depending on experience

Contract Length: 3 months Full Time: 37 hours per week

Herefordshire Council has a fantastic opportunity for a Team Manager to oversee the County and Younger Adults teams for a 3-month contract. This position is pivotal in enhancing adult social care services as outlined by the Care Act 2014. Engaging and impactful, this role involves leading efforts to improve hospital discharge practices, aiming for better outcomes for individuals with complex needs such as learning disabilities, mental health issues, and more. You will interact with partners from health, adult social care, and commissioning to ensure a smooth discharge process. What you will do:

  • Lead the County and Younger Adults teams to enhance hospital discharge practice and processes.
  • Partner with various stakeholders, including health services, social care, commissioning, and Hoople, to ensure timely and effective discharge.
  • Manage complex cases involving learning disabilities, mental health needs, and other challenges.
  • Support the transition of young adults under 25, including care leavers, into adulthood with a focus on achieving improved life outcomes.
  • Oversee the integration of services to facilitate safe discharges from community and mental health hospitals.
  • Drive sustainable change and promote preventative and proactive community services.
  • Offer guidance and support to staff, fostering their development and ensuring service excellence.
