Salary: £36,648 per annum

Main Duties as a Social Worker:
The Integrated Discharge Team works closely with hospital wards to ensure the safe and timely discharge of patients. They provide personalised support to facilitate this process, helping individuals maintain their independence and manage complex needs. By addressing changing circumstances and upholding the principles of The Care Act and the Mental Capacity Act, the team ensures that patients have the highest possible level of choice and control over their care.
- Deliver Professional Social Work Services: Provide comprehensive social work services to adults across Adult Operations and/or Mental Health Services as required, ensuring high standards of care and support.
- Empower Clients: Support clients in maintaining autonomy and making informed decisions about their lives, promoting their choice and control in all aspects of their care.
- Manage Complex Caseloads: Oversee a diverse and complex caseload, demonstrating advanced social work practice skills to effectively assess and manage high-risk situations while balancing support and control to ensure client safety.
- Chair and Facilitate Meetings: Lead various meetings, including Best Interests and Discharge Planning meetings, offering expert guidance and contributing to case conferences. Produce thorough and high-quality assessments and reports.
- Mentor and Support Staff: Exemplify best practices in social work and take on a leadership role in mentoring newly qualified social workers. Provide guidance and support to junior staff, ensuring professional development and adherence to practice standards.
- Provide Expert Support and Advice: Offer expert advice and support in complex cases, utilising your specialised knowledge to address challenging situations and contribute to effective decision-making and case management.
- Conduct Thorough Assessments: Perform detailed assessments of client needs, risks, and circumstances, ensuring that all evaluations are comprehensive and inform effective care planning and intervention strategies.
- Ensure High Standards of Documentation: Maintain accurate and up-to-date records of client interactions, assessments, and case developments, adhering to departmental policies and ensuring the highest quality of documentation.
- Collaborate with Multidisciplinary Teams: Work collaboratively with other professionals, including health and social care colleagues, to deliver integrated support and achieve optimal outcomes for clients.
- Promote Continuous Improvement: Engage in ongoing professional development and contribute to the continuous improvement of social work practices and service delivery within the team.
Key Requirements for this role:
- Must possess a recognised social work qualification
- Registered with Social Work England
- Experience in guiding and mentoring ASYE social workers and students on placement
- A thorough understanding of safeguarding practices for vulnerable adults
- Strong understanding of health and social care legislation, guidance, and policy.
- Effective verbal and written communication skills, able to convey complex information clearly to diverse audiences.
- Proficient in liaising with families, carers, and multidisciplinary teams.
- Competence in maintaining accurate records and using IT systems.
