What you will do:
- Complete Enablement reviews and Care Act assessments to support individuals towards independence.
- Conduct Mental Capacity Act assessments and Best Interests decisions.
- Apply an asset-based approach and collaborate with other professionals for holistic care delivery.
- Complete Continuing Healthcare (CHC) checklists, ensuring a person-centred approach in all evaluations.
- Work efficiently in a fast-paced hospital environment, responding to the evolving needs of service users.
- Maintain up-to-date records in the Local Authority System (LAS) efficiently.
- Travel within the community as needed, requiring driving ability and access to a car for home visits.
Requirements of the Social Worker:
- Hold a degree or equivalent qualification in social work.
- Current registration with Social Work England.
- Demonstrable frontline experience in Social Services within the UK, working as a qualified Social Worker.
- Strong understanding and experience in enablement services and frameworks such as the Mental Capacity Act.
- Proficient in conducting assessments with a focus on person-centred, asset-based approaches.
