Job Title: Locum Social Worker - Hospital Discharge Team

What you will do:

- Liaise with patients, families, and other healthcare professionals to ensure continuity of care.
- Provide expert advice and guidance under the Care Act principles to advocate for individuals' rights.
- Focus on improving patient flow in the hospital and contribute to top-notch hospital performance standards.
We are seeking qualified social workers with a degree or equivalent in social work and registration with Social Work England. Significant frontline experience in social services within the UK while working as a qualified Social Worker is essential.
