What you will do:
- Conduct comprehensive assessments to identify the social care needs of service users.
- Develop, implement, and review care plans in partnership with individuals and their families.
- Work collaboratively with healthcare professionals and other agencies to ensure a coordinated approach to care.
- Provide interventions and support to individuals experiencing mental health difficulties.
- Advocate for the rights and needs of service users, ensuring their voices are heard.

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Social Services within the UK working as a qualified Social Worker
