Location: Tameside

Team/Specialism: Adults Learning Disability, Autism & Transition

Pay Rate: £35 per hour (assignment rate)

Contract: Locum, 12-Months

Hours: Full-time, 36 hours per week. Monday - Friday predominantly, with 6 day contract and occasional duty weekend shifts.

Duties:

  • Complying with Departmental policies and procedures and quality standards
  • Working to the standards of the Service and the corporate standards on customer care and the Citizen’s charter
  • Exercising decision making with reference to eligibility, criteria and prioritising of work
  • Engage in regular supervisions and to develop an annual training plan based on individual needs
  • Develop the service and assist in continuous improvement programmes
  • Maintain a recording system based on using the IAS system and ESCR, which will require keyboard skills
  • Collaborate with colleagues to ensure the most efficient delivery of services across all teams and areas
  • Assess individual’s needs with emphasis on early intervention and prevention and those with more complex needs
  • Engage the customer and carer in the process and be accountable for your own practice
  • Monitor changing need and to re-assess and adjust the support plan and outcomes
  • Produce support plans, which are clearly linked to need, with a clear decision on what the Department will be providing to meet positive outcomes

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
  • Full current UK driving licence and a car available in order to fulfil the duties of the post
  • Good written and analytical skills               
  • Ability to communicate effectively with a wide range of people   
  • Ability to gather information                 
  • Ability to make judgements supported by sound evidence
