What you will do:
- Utilise strength-based approaches to empower individuals and support customers to maximise their potential.
- Work in partnership to assess individual needs, support plan, and arrange a variety of services and support that meet these needs, empowering customer's choice and control.
- Meet the challenges of personalisation and work within financial constraints to develop creative support plans that deliver personalised outcomes and cost-effective solutions.
- Work in partnership to keep people safe and reduce risks. Depending on the role, you may need skills and experience to undertake safeguarding investigations and develop and implement protection plans, as well as assess risk and implement risk management plans.
- Conduct Mental Capacity Assessments with individuals and where appropriate, make best interest decisions.
- Maintain good report writing and record-keeping skills.
- Take part in the development and reviewing of services, focusing on working in partnership with colleagues across all Care Management Teams, Working Age Adults Contracts and Commissioning, and a range of provider organisations to achieve ‘value for money’ through robust assessment and analysis.
- Carry out duty work on a rota basis, supported by lead duty workers.
- Complete CHC checklists and manage care for people with complex health needs.
- All of this work will be supported by your manager, the wider management team, and experienced safeguarding and risk managers who will offer joint work as well as being available for consultation.
