What you will do:
- Conduct assessments and develop support plans to meet the needs of individuals, ensuring they have access to personalised and self-directed support.
- Facilitate reablement services where required, promoting independence and personalisation to help service users assess and manage their own needs.
- Identify and leverage community and support resources, aligning with the Adult Social Care Strategy to maximise individual assets.
- Review service provisions, provide professional guidance, and develop skills for safeguarding work.
- Manage and organise your workload effectively, under the guidance of an Advanced Social Work Practitioner or Team Manager.
- Liaise with local providers and support networks to deliver positive outcomes.
- Contribute to practice and service development while maintaining confidentiality at all times.

Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Social Services within the UK working as a qualified Social Worker
