What you will do:
- Conduct assessments and develop care plans tailored to individual patient needs within the hospital.
- Liaise effectively with healthcare professionals to ensure comprehensive support for each patient.
- Provide crisis intervention and emotional support to patients and their families.
- Facilitate discharge planning to ensure a smooth transition from hospital to community care.
- They will be working from Hospital 3 days a week, working 37 hours a week, Monday to Friday
- Maintain accurate case records and documentation in line with organisational policies.
