What you will do:
- Your role involves working with adults from 18 years and upwards with physical disabilities, learning disabilities, neurodiverse conditions, acquired brain injuries, chronic health conditions and older people
- Conduct thorough strengths-based reviews and assessments, focusing on individual wellbeing.
- Utilise your expertise in social work and relevant legislation to develop and adapt support plans.
- Engage in mental capacity act assessments, ensuring proper support for those in need.
- Collaborate closely with a dedicated team, sharing insights and innovative ideas.
- Use Continuing Health Care checklists and decision support tools for enhanced care provision.
