What you will do:
- Undertake scheduled reviews and monitoring visits to residential and nursing homes.
- Promote and enhance the lives of people living with dementia within residential or nursing placements.
- Work closely with providers to improve the opportunities available to people living with dementia within placement settings.
- Provide detailed reports to the Team Manager regarding visits and interactions with placement settings.
- Collaborate effectively with other professionals and health colleagues to ensure comprehensive support for older adults.
